Search PetHealthAlert.com Tag ID 1234 above for an example of how it works, and for examples of what information you should Register.

You can register as much or as little information as you like.

The "Pet Medication" field can include any medications your pet receives and how often, and can include monthly heartworm or flea prevention, vitamins or supplements.

The "Emergency Contact" field should include at least 2 emergency contact telephone numbers, or instructions how to reach you if your pet is lost.

The "Important Pet Information" field is a good place to Register your pet's medical history, surgical history, or vaccination history. You can also include any vital information about your pet, for example, "does not like children", "good with other dogs", "will run away off leash!", or "allergic to wheat products".
